Cystoid Macular Edema: A Comprehensive Guide to Medical and Surgical Management
What is Cystoid Macular Edema?
Cystoid macular edema (CME) is a condition that causes swelling in the macula, the central part of the retina. The macula is responsible for central vision, which allows us to see fine details and colors. CME can lead to blurred vision, distorted vision, and even vision loss.

CME is a common complication of eye surgery, especially cataract surgery and glaucoma surgery. It can also be caused by other conditions, such as diabetes, uveitis, and retinal vein occlusion.
Symptoms of Cystoid Macular Edema
The symptoms of CME can vary depending on the severity of the swelling. Some people may only experience mild blurred vision, while others may have severe vision loss. Common symptoms of CME include:
* Blurred vision * Distorted vision * Loss of central vision * Difficulty reading * Difficulty seeing fine details * Colors may appear washed out or faded
Causes of Cystoid Macular Edema
CME is most commonly caused by eye surgery, especially cataract surgery and glaucoma surgery. Other causes of CME include:
* Diabetes * Uveitis (inflammation of the uvea, the middle layer of the eye) * Retinal vein occlusion (blockage of a vein in the retina) * Trauma to the eye * Certain medications, such as corticosteroids
Diagnosis of Cystoid Macular Edema
CME is diagnosed based on a comprehensive eye exam. Your doctor will dilate your pupils and examine the retina using a special microscope called an ophthalmoscope. Your doctor may also order an optical coherence tomography (OCT) scan. An OCT scan is a non-invasive imaging test that can create detailed cross-sectional images of the retina.
Treatment of Cystoid Macular Edema
The treatment of CME depends on the underlying cause and the severity of the symptoms. Medical treatment options include:
* Nonsteroidal anti-inflammatory drugs (NSAIDs) * Corticosteroids * Anti-VEGF medications * Laser therapy
Surgical treatment options include:
* Pars plana vitrectomy * Macular translocation
The best treatment option for you will depend on your individual circumstances. Your doctor will discuss the risks and benefits of each treatment option with you and help you make the best decision for your care.
Prognosis for Cystoid Macular Edema
The prognosis for CME varies depending on the underlying cause and the severity of the symptoms. With early diagnosis and treatment, most people with CME can achieve good visual outcomes. However, some people may experience permanent vision loss.
Prevention of Cystoid Macular Edema
There is no sure way to prevent CME, but there are some things you can do to reduce your risk of developing this condition. These things include:
* Controlling your blood sugar if you have diabetes * Getting regular eye exams * Avoiding eye trauma * Telling your doctor about any medications you are taking, especially corticosteroids
